Abstract
Background: Preeclampsia is a major contributor to maternal morbidity and mortality. The application of echocardiography in evaluating women with preeclampsia is so informative about physiological changes triggered by the condition's stress.
Objective: To assess the severity of cardiac disease involvement in pregnant women with severe preeclampsia using echocardiography.
Patients and Methods: A case-control study was carried out in the Department of Obstetrics and Gynaecology throughout the period January to June 2024. A convenient sampling method was adopted to enrol 100 pregnant women who presented during the third trimester of pregnancy. The sample included three groups: The preeclampsia group (25) pregnancy-induced hypertension without proteinuria (25), and the control group (50 normotensive). Echocardiographic was done in the Department of Internal Medicine. The statistical package of social science (SPSS) version 26 was used for data entry and analysis. A P-value of ≤ 0.05 was considered statistically significant.
Results: The preeclampsia group had increased interventricular septal wall thickness during diastole. Preeclamptic pregnant women and hypertensive women without proteinuria have a higher left ventricular mass and left ventricular mass index by height than normotensive pregnant women. In the preeclampsia group, 32% of patients had concentric hypertrophy, and 12.0% had eccentric hypertrophy, these proportions of patients were significantly higher than in the other two groups.
Conclusions: Women with severe preeclampsia have elevated left ventricular systolic pressure values, diastolic dysfunction, diminished right ventricle strain, and abnormal cardiac remodelling when compared with normotensive pregnant control patients.
